Disclosure

This program is NOT insurance coverage, not intended to replace insurance, and does not meet the minimum creditable coverage requirements under the Affordable Care Act or Massachusetts M.G.L. c. 111M and 956 CRM 5.00.


It contains a 30-day cancellation period policy, provides discounts only at the offices of contracted health care providers, and each member is obligated to pay the discounted medical charge in full at the point of service.

Recuro Health does not guarantee that a physician will write a prescription, and physicians do not prescribe DEA controlled substances, non-therapeutic drugs and certain other drugs which may be harmful because of their potential for abuse. Recuro Health affiliated physicians reserve the right to deny care for potential misuse of services. Medical services provided by physicians are subject to their professional judgment. Recuro Health operates subject to state regulation and some services may not be available in certain states. Urgent Care services are available for all ages. Virtual Primary Care is only available to members 18 and older. Behavioral Health is available for members 14 and older.


ManifestRx is a prescription cost-saving program and is not an insurance provider. It is not intended to replace insurance coverage. Discount percentages may vary depending on the medication and pharmacy location. For mail orders of maintenance medications, each person is entitled to 6 free shipments per year. For urgent care medications, there is a maximum allowance of $250 per person per year.


Aetna Dental Access: The discount program provides access to the Aetna Dental Access® network, which is administered by Aetna Life Insurance Company (ALIC). ALIC does not offer or administer the discount program, and is not an affiliate, agent or principal of the discount program. Dental providers are independent contractors and not employees or agents of ALIC. ALIC does not provide dental care or treatment and is not responsible for outcomes.
 

Lab Testing Benefit Services are not available in the following states:
Quest: AZ, NJ, NY, and RI.
At Home Tests: NJ, NY, and RI

Personal information we collect about members comes from one or more of the following sources: paper application, faxed application, web application, your employer, membership association, insurance carrier, or affinity groups.

Personal information collected includes, but is not limited to:

• Name

• Mailing address

• Telephone numbers

• E‑mail address

• Social security number (when necessary)

• Date of birth

• Gender

• Geolocation

• Mobile device information

• Dependent name, gender, and date of birth

• Dependent social security number (when necessary)

• Dependent relationship

• Bank account or credit card information


Sharing Information with Affiliates and Service Providers

New Benefits will share member information with its affiliates, related companies, and service providers to the extent necessary to provide and manage benefits. New Benefits will not share, sell, or otherwise disclose member information to unrelated third parties for marketing purposes.


Requests to Know, Correct, or Delete

You have the right to request disclosure, correction, or deletion of your personal information, subject to certain exceptions. To submit such a request, call 800.800.7616 or email info@newbenefits.co.


Right to Opt‑Out and Non‑Discrimination

New Benefits will not sell personal information and you have the right not to be discriminated against for exercising your privacy rights.


Authorized Agents

An authorized agent may make privacy requests on your behalf with proof of authorization.


Response Time

New Benefits will confirm receipt of a privacy request within 10 business days and respond within 45 days of receipt.

Changes to the Privacy Notice
